    • ONE (1) month2 for GLC, PLC, MNC and GovernmentOR
    • THREE (3) months2 together with the salary crediting statement for non GLC, PLC, MNC and Government; OR
  • Latest interim EPF statement

If Commission Earner
 

  • Copy of MyKad (front and back)1; AND
  • Copy of Driving License ; AND

  • Latest SIX (6) months salary slip with crediting bank statements; OR
  • Latest interim EPF statement; OR
  • Latest Form B/BE (with validated payment receipt to LHDN)

If Self Employed / Business Owners
 

  • Copy of MyKad (front and back)1; AND
  • Copy of Driving License ; AND
  • Business Registration Certificate3

 

Any ONE of the following documents:

 

  • Latest SIX (6) months bank statement; OR
  • Latest Form B/BE (with validated payment receipt to LHDN)
Documents Required - Business / Non Individual Applicants

Sole Proprietor / Partnership

 

  • Copy of Business proprietor/partner(s)’s MyKad (front and back); AND
  • Business Registration Certificate and Form D; AND
  • Financial Report for previous year; AND
  • Latest SIX (6) months bank statements

Private Limited / Sdn Bhd

 

  • Copy of director/guarantor’s MyKad (front and back); AND
  • Business Registration Certificate - Form 24 & 49; AND
  • Memorandum and Articles of Association (M&A) ; AND
  • Certificate of Incorporation - Form 9 & 13; AND
  • Financial Report for previous year; AND
  • Latest SIX (6) months bank statements

NOTE:


  1. If the application requires a guarantor, both applicant and guarantor shall provide a copy of MyKad (front and bank)
  2. The Bank may require up to 6 months of salary slips and income statements if the income comprises of variable components such as overtime, commissions, etc. in addition to fixed salary.
  3. Business registration document as per below:
  • Sole Proprietor & Partnership - Form A & D; OR
  • Private Limited / Sdn Bhd - Form 24 & 49; OR
  • East Malaysia - Trading License
